• Room and Board Comfortable living accommodations in a safe and nurturing environment.
  • Qualifications Age 18+, ID/DD diagnosis, Medicaid waiver eligible
  • Scope Private/shared rooms, common areas, meal plans, maintenance, housekeeping
  • Requirements Intake assessment, agreement to house rules
• Transportation Assistance Support for accessing medical appointments, community activities, and vocational programs.
  • Qualifications Resident of our group home, medical necessity
  • Scope Scheduled transport to appointments and activities, accessible vehicles, staff accompaniment
  • Requirements Pre-scheduled appointments, authorization for transport
• Life Skills Training Structured opportunities for skill development in daily living activities, vocational training, and recreational activities.
  • Qualifications Resident of our group home, ISP indicating need
  • Scope: Training in personal care, meal preparation, household management, vocational skills, and social activities
  • Requirements Participation in goal-setting, consistent attendance
• Healthcare Coordination Coordination of medical care, therapy services, and medication management.
  • Qualifications Resident of our group home, ISP with healthcare needs
  • Scope Medical appointment scheduling, medication management, therapy services
  • Requirements Consent for medical information sharing, regular health assessments
• 24/7 Supervision and Support Round-the-clock supervision and personalized support.
  • Qualifications Resident of our group home, assessment indicating need
  • Scope Continuous on-site staff, emergency response, daily activity support
  • Requirements Comprehensive care plan, communication with family/guardians
